Registration Information

 

TO REGISTER: Advance registration is required for all activities either by mail, fax, or in person. We cannot accept registrations by phone. Registrations are accepted on a first-come first-serve basis. Registrations received after the deadline are accepted on a space available only basis and are subject to a late fee. Payment in full is due at the time of registration. Checks should be made payable to Trinity United Methodist Church.

WAIVER OF LIABILITY AND RELEASE

The undersigned parent/legal guardian acknowledges there are inherent risks involved with the participation in youth sports and other recreational activities and that Trinity United Methodist Church will not be held responsible for any injury that may occur during participation in these activities. The undersigned acknowledges that his/her child’s participation is voluntary and on his/her own behalf and on behalf of the child and the child’s heirs and legal representatives does hereby release Trinity United Methodist Church and its employees, agents and contractors from any claims for injury, death, loss or other damages that may occur as a result of participation and agrees and covenants not to commence or prosecute any claim or cause of action based on any matter constituting a released claim.
